ON THE DAY OF THE ACCIDENT, AIRCRAFT ^66-16243^ WAS INVOLVED IN THE DIVISION READY REACTION FORCE. THE ACCIDENT OCCURRED ON THE FIRST LIFT INTO THIS PARTICULAR LZ. SINCE THIS WAS AN ^ARVN^ OPERATION THE ACL WAS SEVEN TROOPS. ^LZ CAROL^ IS A TWO SHIP LZ, HOWEVER, DUE TO ELEVATION (3120') AND THE FACT THAT THE LZ WAS A PINNACLE, A SINGLE SHIP OPERATION WAS PLANNED WITH THE AIRCRAFT INVOLVED IN THE ASSAULT LANDING AT 15 SECOND INTERVALS. FIVE AIRCRAFT TOOK PART IN THE OPERATION. ^66-16243^ PILOTED BY ^WO RAGAN^ WAS TO BE THE THIRD TO LAND. THE FLIGHT FROM THE PZ TO THE LZ WAS WITHOUT INCIDENT, EXCEPT FOR A RADIO MESSAGE FROM THE LEAD AIRCRAFT TELLING THE REST OF THE FLIGHT TO BE CAREFUL OF DEBRIS SCATTERED THROUGHOUT THE LANDING ZONE. BOTH CHALK 1 AND 2 LANDED AND DEPARTED FROM THE LZ WITHOUT ANY PROBLEMS. CHALK 3 MADE A NORMAL APPROACH TO THE LZ AND PULLED 35 POUNDS OF TORQUE PRIOR TO TOUCHDOWN. AS THE SKIDS OF THE AIRCRAFT WERE JUST ABOUT ON THE GROUND THE PASSENGERS QUICKLY DEPARTED THE AIRCRAFT WITHOUT ANY WORD FROM THE CREW. THIS CAUSED THE AIRCRAFT TO BECOME UNSTABLE AND BEGIN ROCKING FROM SIDE TO SIDE. WHEN THIS HAPPENED ^WO RAGAN^ ABRUPTLY PULLED UP ON THE COLLECTIVE CAUSING THE AIRCRAFT TO COME UP TO A THREE FOOT HOVER. THE SUDDENNESS WITH WHICH THE AIRCRAFT COMMANDER PULLED UP ON THE COLLECTIVE CAUSED THE N2 RPM TO DROP AS THE DROOP CAM COMPENSATOR COULD NOT KEEP UP WITH THE ABRUPT INCREASE IN POWER. THE AIRCRAFT BEGAN TO YAW TO THE RIGHT WHICH GAVE ^WO RAGAN^ THE IMPRESSION THAT HE HAD A TAIL ROTOR MALFUNCTION. AT THIS TIME, HE ATTEMPTED A HOVERING AUTOROTATION WITH THE AIRCRAFT LANDING HARD ON THE RIGHT SKID, LEVELING AND FINALLY COMING TO REST. A "MAYDAY" MESSAGE WAS THEN SENT OUT AND THE AIRCRAFT WAS SHUT DOWN.\\

Accident Summary:
DURING CLIMB OUT FROM TAKEOFF AT APPROXIMATELY 50 FEET AGL A BANG WAS HEARD FROM TRANSMISSION COMPARTMENT AND THE AIRCRAFT YAWED MOMENTARILY TO THE RIGHT. AIRCRAFT COMMANDER IMMEDIATELY BEGAN TO LOOK FOR A PLACE TO LAND THE AIRCRAFT BECAUSE ALTITUDE COULD NOT BE MAINTAINED. AIRCRAFT LANDED IN PERIMETER DEFENSES CAUSING DAMAGE TO THE SYNCRONIZED ELEVATOR. THE MAIN ROTOR BLADES FLEXED DOWN ON LANDING AND SERVED THE REAR SECTION OF THE TAIL ROTOR DRIVE SHAFT.\\
