Accident Summary:

The aircraft and crew departed Long Binh on a training mission for the purpose of providing LT Weaver his initial Vietnam flight orientation which included practice of emergency procedures. The instructor pilot was demonstrating a straight-in autorotation when he allowed the tail rotor to strike the ground causing major damage to the aircraft. There were no injuries

Accident Summary:

AIRCRAFT DEPARTED PHAN RANG DB AT APPROXIMATELY 0945 ENROUTE TO DALAT. ABOUT 20 NM NW OF PHAN RANG A LOUD NOISE WAS HEARD IN THE TAIL PORTION OF THE AIRCRAFT, AT THIS TIME THE AIRCRAFT COMMANDER TOOK THE CONTROLS. THE AIRCRAFT SHUTTERED AND VIBRATED MODERATELY TO SEVERLY. THE ALTITUDE WAS 3,000 FT AGL, AIRSPEED 100 KTS. SEVERE VIBRATIONS WERE FELT THROUGH THE PEDALS AND NO YAW MOVEMENT WAS EVIDENT WHEN THE PEDALS WERE PUSHED. AIRSPEED WAS REDUCED TO 85-90 KNOTS TO ELIMINATE SOME OF THE VIBRATION. HOWEVER, AT 30-35 PSI AND 85-90 KNOTS THE AIRCRAFT COULD NOT HOLD ALTIUTDE AND WAS PITCHED TO ONE SIDE; THUS THE AIRCRAFT COMMANDER ANALYSED THE PROBLEM AS A COMPLETE LOSS OF TAIL ROTOR. HE THEN ATTEMPTED TO AUTOROTATE TO A ROAD AT 80-85 KNOTS. THE AIRCRAFT WAS COCKED TO ONE SIDE AND UPON NEARING THE ROAD IT WAS DECELERATED TO 75 KNOTS, STILL COCKED TO ONE SIDE, IT DRIFTED FROM THE ROAD TO A TREE LINE. POWER AND FORWARD CYCLIC WERE APPLIED BUT THE AIRCRAFT FAILED TO ACCELERATE. THE NOSE CONTINUED TO COME UP AND THE AIRCRAFT DRIFTED FURTHER FROM THE ROAD AT WHICH TIME THE AIRCRAFT COMMANDER AGAIN WENT INTO AUTOROTATION. AT 60 KNOTS THE NOSE CONTINUED UP AND THE AIRCRAFT BEGAN SPINNING OUT OF CONTROL. AT 40 KNOTS THE SPIN WAS STILL VIOLENT (SPINNING LEFT) AND AT TREETOP LEVEL, AT THIS TIME BACK PRESSURE WAS APPLIED TO THE CYCLIC ZEROING OUT THE AIRSPEED. AN ATTEMPT TO CUSHION THE AIRCRAFT WAS MADE AS IT BEGAN TO SETTLE INTO THE TREES STILL IN A SLIGHT LEFT SPIN, LANDING UPRIGHT BUT EXTREMELY HARD. ALL PERSONNEL ON BOARD EXITED THE AIRCRAFT; THERE WAS NO POSTCRASH FIRE. A DEFENSIVE PERIMETER WAS SET UP AROUND THE AIRCRAFT, PERSONNEL FROM A NEARBY ENGINEER UNIT REACHED THE WRECKAGE SHORTLY THEREAFTER.\\
