Accident Summary:

ON 14 OCTOBER 1971, ARMY AIRCRAFT UH-1H, SERIAL NUMBER^68-15688,^ WAS THE TRAIL IN A FLIGHT OF TWO. THE TWO AIRCRAFT WERE RESUPPLYING AN OUTPOST OF THE ^2ND BATTALION, 501ST INFANTRY, 101ST AIRBORNE DIVISION^ LOCATED ON A PINNACLE 5 MILES SOUTHWEST OF DONG BA THIN, RVN, COORDINATES BP 978215. THE FIRST AIRCRAFT RECONNED THE LZ AND STARTED HIS APPROACH. ON FINAL, THE FIRST AIRCRAFT MADE A GO AROUND BECAUSE OF THE WINDY CONDITIONS. HE THEN ATTEMPTED ANOTHER LANDING INTO THE LZ USING AN APPROACH PATH 180 DEGREES OFF HIS FIRST APPROACH HEADING. HE LANDED WITH NO DIFFICULTY. AFTER DEPARTING THE LZ, THE MEN ON THE GROUND POPPED SMOKE FOR AIRCRAFT ^68-15688.^ IT APPEARED TO THE PILOT THAT THAT THE WIND HAD CHANGED 180 DEGREES FROM THE LEAD SHIP'S APPROACH AXIS. THE PILOT ENTERED THE APPROACH USING THE SMOKE TO DETERMINE THE LANDING DIRECTION. ON SHORT FINAL, HE BEGAN TO ADD POWER AND RAN OUT OF LEFT PEDAL. HE THEN LOST DIRECTIONAL CONTROL AND BEGAN TO SPIN TO THE RIGHT. THE AIRCRAFT COMMANDER TOOK THE CONTROLS BUT THE AIRCRAFT CONTINUED TO SPIN TO THE RIGHT MAKING 2 OR 3 REVOLUTIONS. DUE TO THE FLARED ATTITUDE OF THE AIRCRAFT, THE TAIL BOOM WAS LOW AND AS THE AIRCRAFT SPUN IT SEVERED AT THE SYNC-ELEVATOR. THIS RESULTED IN A RADICAL OUT-OF-CG CONDITION CAUSING THE NOSE TO DIP DOWN AND JERK TO THE RIGHT. THE MAIN ROTOR BLADES STRUCK THE GROUND THROWING PARTS ALL OVER THE LZ AND THE AIRCRAFT CAME TO REST ON ITS RIGHT SIDE.\\
